Medical Services for Pediatric Mental Health

These Terms of Service (“Terms”) govern your use of medical and behavioral health services provided by Grizelda M. Anguiano, M.D., PLLC, doing business as Anchored (“the Practice,” “we,” “our,” or “us”). By scheduling an appointment, completing intake forms, or receiving services, you agree to these Terms.

If you do not agree with any part of these Terms, you should not proceed with services.

1. Scope of Services

Dr. Grizelda Anguiano is a board-certified pediatrician offering medical care with a focus on mental health and wellness.

Services may include:

- Mental health and behavioral evaluations

- Medication management

- ADHD assessments (including QbCheck or other tools)

- Family-focused care plans

- Parenting support and behavioral guidance

- Referrals to therapy or allied services as needed

Medical services are provided under a self-pay, direct care model and are not billed to insurance.


Clinical documentation may be supported by secure AI tools (e.g., Heidi.ai) to improve record accuracy and continuity of care.

2. Eligibility

Services are available for children, adolescents, and young adults, and may include parent or family involvement. A legal guardian must consent to treatment for any minor under 18 years of age.

3. Relationship and Responsibility

Receiving services from the Practice establishes a patient-provider relationship governed by state and federal medical laws.

You agree to:

- Provide accurate and complete health history

- Actively participate in treatment planning

- Follow medical advice and safety guidance

- Ask questions and communicate concerns in a timely manner

- Maintain respect and courtesy for all staff members

- Participate in family support strategies when appropriate

4. Appointments and Cancellations

Appointments are scheduled by request and subject to availability.

You agree to:

- Arrive on time or provide notice if you will be late

- Cancel or reschedule with at least 24 hours’ notice

- Understand that missed or late cancellations may incur a fee

- Notify the Practice promptly of emergencies that impact attendance

We reserve the right to reschedule or cancel appointments due to clinical or administrative needs.

5. Payment and Fees

The Practice operates on a private-pay basis only. Payment is due at the time of service. We do not submit claims to insurance, but you may request a superbill for reimbursement when applicable.

Fees are clearly listed on our website or intake forms and may include:

- Initial consultations and follow-ups

- ADHD testing (e.g., QbCheck)

- Behavioral and wellness assessments

- Family or care coordination visits

- Optional care plan access subscription ($125/month)

All fees are subject to change with prior notice.

6. Confidentiality and Privacy

We are committed to safeguarding your Protected Health Information (PHI). We comply with HIPAA regulations and maintain administrative, physical, and technical safeguards. Please refer to our Notice of Privacy Practices for details about how your PHI is used and protected.

Information may be disclosed without additional consent only as required by law or to prevent imminent harm.

Clinical documentation tools (e.g., Heidi.ai) may use real-time transcription or AI-supported services to assist with record accuracy. These tools are HIPAA-compliant and do not store recordings or identifiable information without your written consent.

7. Technology and Communications

We may use secure platforms (e.g., PracticeQ, Zoom for Healthcare, encrypted email, or documentation tools such as Heidi.ai) to facilitate care, reminders, and communication. These tools are HIPAA-compliant. By using our services, you consent to these methods unless you opt out in writing.

8. Medical Records and Requests

You may request access to or copies of your medical records by submitting a written request. We may charge a reasonable fee for records processing and delivery.

Requests will be fulfilled in accordance with state and federal law, typically within 15 business days unless an extension is necessary.

9. Termination of Services

You or the Practice may terminate the relationship at any time.

We reserve the right to discontinue services if:
- There is non-payment for services
- Appointments are repeatedly missed
- There is a breakdown in mutual trust
- Services fall outside the scope of care we provide

If services are terminated by the Practice, we will provide at least one appropriate referral or summary of care upon request.

10. Disclaimers

While we strive to provide evidence-based, comprehensive care, mental health outcomes are shaped by many factors—such as engagement in care, adherence to recommendations, family dynamics, and environmental stressors. We cannot guarantee specific results, but we are committed to working collaboratively with you toward progress.

11. Governing Law

These Terms are governed by the laws of the State of Texas. Any disputes arising from the use of our services shall be subject to the jurisdiction of Texas courts.
